This "Certification of Birth" was issued by the New York State Department of Health. As noted here, Holmes Sterling Morrison, Jr. was born on August 29, 1942 at Meadowbrook Hospital in East Meadow, New York.  This certificate was issued seventeen years later, on June 22, 1959, probably to replace a lost original certificate.
This was owned by Sterling Morrison until his death in 1995; we acquired it from his widow, Martha Morrison, who's letter of authenticity is included, as is Recordmecca’s written lifetime guarantee.
